I mean, when you're some megacorp, your AT&T bill is at least tens of millions per year, and probably split across hundreds of cost centers. It's real easy for stuff to slip through the cracks, especially if they're not seeing big data usage. They might even have a contract where the per-active sim cost is very small and they pretty much only pay for the data usage.
