@foone I worked in medical records years ago and ambient medical documentation is a reasonable, though ambitious goal.
The idea is that doctors and nurses are busy and wouldn't it be nice if their work was self-documenting? Like they do a thing and it's automatically recorded in a useful form. A recording isn't useful, you want a summary.
Similarly you want the record to also be available as part of the environment. Ask a question and the computer answers, or have it interject when appropriate
